Please join us in Houston on Nov. 8-9, 2007, for
Chronic Illness and Disabilities: Transition from Pediatric to Adult Based Care,
a conference that offers the latest information on transitioning youth and young adults with chronic illness and disability to adult-based services.
In its eighth year, this conference offers
a unique opportunity to hear from national and local clinical and
family experts on topics and issues associated with adolescent
transition.
- Physicians, clinicians and
other health care professionals working with youth and young
adults with chronic Illness: Topics will be covered that are
applicable to both primary and specialty care practices. You'll
hear an interdisciplinary perspective on how teams can come
together to facilitate the important transition from adolescence
to adulthood in health care.
This activity has been approved for AMA PRA Category 1 Credit for
physicians, nurses, physical and occupational therapy and social work.
- Parents, teens and young adults:
The conference offers an opportunity to hear practical medical,
legal and insurance information.
For both clinicians and families, the
conference will offer time to meet and talk with faculty, to
exchange ideas among participants, including experienced clinicians,
parents, and young adults, and to share knowledge and insight about
how best to make a successful transition to adulthood.
