• Ambulatory Care: Pulmonary Clinic Rotation

    Rotation Site Description

    The Pulmonary clinic is a longitudinal experience that will take place in the Clinical Care Center of Texas Children’s Hospital. The resident will participate in a multidisciplinary approach to care for pulmonary patients in an ambulatory clinic setting. The majority of patients cared for in this clinic are cystic fibrosis patients. Pediatric pulmonologists specializing in cystic fibrosis treatment are the primary care givers; however, dietitians, social workers, nurses, respiratory therapists, and pharmacists are actively involved in patient care.

    Rotation Responsibilities

    Provide clinical pharmacy services to pediatric pulmonary patients, which includes:

    • Understanding the basic pathophysiology of pediatric pulmonary diseases, specifically cystic fibrosis, and associated morbidities
    • Understanding the pharmacology of each medication prescribed/dispensed
    • Obtaining and reviewing the medication history and ensuring appropriate dosing for each drug used based on height, weight, BSA, and renal and hepatic function
    • Counseling patients regarding prescription and non-prescription medication regimens as necessary/indicated by the provider
    • Counseling patients regarding drug/drug or food/drug interaction as necessary/indicated by the provider
    • Reporting of adverse drug events and medication incidents/errors
    • Interacting effectively with members of the multidisciplinary team
    • Providing medication-related in-services to the clinic nursing staff
       

    Activities

    Pulmonary Clinic: Mondays, 12:00- 5:30 pm (or until clinic closes)

    Required Meetings: Meetings as scheduled with preceptor.

    The resident will be expected to provide nursing and pharmacy in-services during the clinic period.

    Excused absences are limited to clinic holidays, professional meetings, and pre-approved vacations. The resident will be evaluated quarterly.
